DHA better than EPA for the prevention treatment of liver disease


Research at Oregon State University has found that one particular omega-3 fatty acid has a powerful effect in preventing liver inflammation and fibrosis – common problems that are steadily rising along with the number of Americans who are overweight.

The American Liver Foundation has estimated that about 25 percent of the nation’s population, and 75 percent of those who are obese, have nonalcoholic fatty liver disease. This early-stage health condition can sometimes progress to more serious, even fatal diseases, including nonalcoholic steatohepatitis, or NASH, as well as cirrhosis and liver cancer.

The study, published online in the Journal of Nutrition, was one of the first to directly compare the effects of two of the omega-3 fatty acids often cited for their nutritional value, DHA and EPA.

In research with laboratory animals, it found that EPA had comparatively little effect on preventing the fibrosis, or scarring, that’s associated with NASH. However, DHA supplementation reduced the proteins involved in liver fibrosis by more than 65 percent.

“A reduction of that magnitude in the actual scarring and damage to the liver is very important,” said Donald Jump, a principal investigator with the Linus Pauling Institute at OSU and a professor in the College of Public Health and Human Sciences.

“Many clinical trials are being done with omega-3 fatty acids related to liver disease,” Jump said. “Our studies may represent the first to specifically compare the capacity of EPA versus DHA to prevent NASH. It appears that DHA, which can also be converted to EPA in the human body, is one of the most valuable for this purpose.”

The issues have taken center stage as the weight of Americans continues to rise, with a related increase in the incidence of fatty liver disease and liver damage.

NASH is a progressive form of liver disease that is associated with chronic inflammation and oxidative stress, resulting from excess fat storage in the liver. Chronic inflammation can eventually lead to fibrosis, cirrhosis, or even liver cancer. While management of lifestyle, including weight loss and exercise, is one approach to control the onset and progression of fatty liver disease, other approaches are needed to prevent and treat it.

About 30-40 percent of people with nonalcoholic fatty liver disease progress to NASH, which in turn can result in cirrhosis, a major risk factor for liver cancer. While this research studied the prevention of fatty liver disease, Jump said, ongoing studies are examining the capacity of DHA to be used in NASH therapy.

The levels of omega-3 oils needed vary with the health concern, officials say.

“Omega-3 fatty acids are typically recommended for the prevention of cardiovascular disease,” Jump said. “Recommended intake levels of omega-3 fatty acids in humans for disease prevention are around 200-500 milligrams of combined DHA and EPA per day.”

Levels used in therapy to lower blood triglycerides, also a risk factor for cardiovascular disease, are higher, about 2-4 grams of combined EPA and DHA per day. The OSU studies with mice used DHA at levels comparable to the triglyceride therapies.

“DHA was more effective than EPA at attenuating inflammation, oxidative stress, fibrosis and hepatic damage,” the researchers wrote in their conclusion. “Based on these results, DHA may be a more attractive dietary supplement than EPA for the prevention and potential treatment of NASH in obese humans.

Keep the skin cancer from brain disease

Of course not everyone wants to have skin cancer. Everyone must try to avoid and prevent it. But a new study suggests that having a non-melanoma skin cancer may reduce the risk of Alzheimers disease.

Study participants who had been diagnosed with non-melanoma skin cancer is known to have a 80 percent lower risk of developing Alzheimers compared with those who did not have skin cancer, according to researchers at Albert Einstein College of Medicine in New York.

The results obtained after the researchers observed 1,102 people aged 70 years or older during 3.7 years. They check the health of the skin associated with cancer and Alzheimers disease brain, as reported by Live Science.

At baseline, no participants who have dementia or Alzheimers, while da 109 people diagnosed have non-melanoma skin cancer before.

During the research, it is known that 32 people had one type of skin cancer. While 126 people were known to develop dementia. Researchers found that there were only two participants with non-melanoma skin cancers are developing Alzheimers disease.

Even so researchers did not observe the possibility of a link between melanoma and Alzheimers because not enough participants who had the skin cancer, said Dr. Richard Lipton of the Albert Einstein College of Medicine.

Results of this study does not mean you have to avoid getting skin cancer to the brain such as Alzheimers disease or dementia. It would be very nice if you could protect yourself from both diseases. still use sunscreen when traveling in the sun and doing a healthy lifestyle and continue to hone the sharpness of the brain to ward off brain disease.

Beets could lower risk of heart disease


New research from the University of Reading has shown that eating four slices of bread containing beetroot lowers blood pressure and improves the function of blood vessels, significantly improving heart health.

The researchers provided 24 participants with four slices (200g) of bread containing 100g of beetroot, or control bread with no beetroot enrichment once on two separate occasions. They found the diastolic¹ blood pressure of those who consumed ‘beetroot bread was lowered by up to 7 mmHg when compared to the control group approximately 3 hours after consumption.

Evidence suggests that a reduction in diastolic blood pressure of 5-6 mmHg over a five year period could reduce the chances of a stroke by 38% and coronary heart disease by 23% less. In addition, prolonged high blood pressure is an important risk factor for the development of heart disease, which is the single biggest killer in the UK, causing nearly 179, 000 deaths per year.

The component of beetroot bread thought to be responsible for the beneficial effects on blood vessel function and blood pressure is dietary nitrate. Dietary nitrate is a natural component of beetroot and a number of other vegetables such as spinach, lettuce and rocket.

When dietary nitrate is eaten it produces nitric oxide in the blood vessel wall which causes relaxation of the vessel and increased blood flow. This ultimately results in lowering of blood pressure and an improvement in blood vessel function.

Julie Lovegrove, Professor of Human Nutrition, Head of the Hugh Sinclair Unit of Human Nutrition and the Deputy Director of the Institute for Cardiovascular and Metabolic Research (ICMR) at the University of Reading, who led the study said: "These exciting and novel findings show for the first time that bread containing beetroot improves blood vessel function. This is an important addition to the increasing body of evidence that suggests beneficial effects of dietary nitrate rich foods on the heart.

"This research also supports the findings of our previous study which was carried out last year and showed that beetroot in the form of juice or bread lowers blood pressure. Collectively, these studies suggest a potential role for foods rich in dietary nitrate in the management of high blood pressure."

The study, published in the Journal of Nutrition, was undertaken by Dr Ditte Hobbs in the University of Readings Hugh Sinclair Human Nutrition Group, which has an international reputation for its research into the relationship between diet and the risk of chronic disease such as cardiovascular disease, diabetes, neurodegenerative disease and cancer.

Women Cholesterol and Heart Disease Three of My Favorite Things

These are a few of my favorite thinnnngs.  Sorry, nostalgia.

Women are not men. That should be obvious to everyone. Women have a different balance of sex hormones than men do, they have a higher body fat requirement, they can grow a baby in their bellies for 9 months, and they really like romantic comedies. Despite their inexplicable love for Matthew McConaughey movies, however, the scientific research world likes to pretend these differences dont exist. Over my years of studying cholesterol and cardiovascular disease research, Ive noticed a bias here. But its time to wise up... the research on cholesterol and heart disease shows a stark difference between the sexes. If youre a woman, and theres a 50% chance that you are, then youve been fed a load of shit. Heres why.

Cholesterol Levels in Women
In men, there has historically been an association between cholesterol levels and risk of heart disease.  Take the MRFIT trial for example, one of the landmark studies in this area... (1)


If youre a middle-aged man, risk of heart disease and dying overall increases with your cholesterol level.  Theres SO much more to it than just this, of course... theres HDL, LDL, triglycerides, and most importantly LDL particle number. All of these are more important markers than total cholesterol in my opinion, and each case should be evaluated on an individual basis.  Just because your cholesterol is a little high doesnt mean youre going to die.  But for the purposes of this article, you cant deny the relationship above.  It exists. 

It exists in men, that is. Women? Not so much. Take the Norwegian HUNT 2 Study, for example (2). The researchers followed 52,087 Norwegians aged 20-74 who were free of cardiovascular disease (CVD) for 10 years, then assessed the relationship of total cholesterol with total mortality, CVD mortality, and heart disease mortality. Most importantly to the purposes of this article, they studied both men and women, and they analyzed each of their data separately.  As youve probably already assumed, they each yielded very different results.

Note: Since this study comes from Europe, the units for blood cholesterol are shown in mmol/L, rather than the mg/dL that we are used to. The researchers classified the participants into four groups, based on their blood cholesterol. Here are the converted unit values in mg/dL for the four groups... <193, 193-229, 230-269, and >270.

This is the all-cause mortality data, the most important information from the study.  Just take a moment and compare these graphs to the MRFIT graph above.  Do it.  For men, we see a similar U-shaped trend, although this study shows a lesser risk with high cholesterol than with low.  But just look at the women... those with the highest total cholesterol, over 270, were least likely to die! And thats statistically significant. Of course there are other studies that show this gender difference as well (3).

The relationship between cholesterol and risk of imminent death clearly differs between men and women.

Women and Cholesterol-Lowering Drugs
Ive written of cholesterol-lowering drugs (aka statins) a couple times, once about the potential side effects and once concerning their efficacy (4, 5).  The truth is, statins dont save any lives in women, and the potential side effects make these drugs a definite no-go.  I know its hard to believe; these drugs are given out to women like candy, just like they are to men.  On top of that, your doctor likely isnt aware theres any difference between sexes.  Thats why I provide evidence.  Prepare to be mindblown.

The most positive, statin-supporting study ever done is known as ASCOT-LLA (6).  It was a secondary prevention study, meaning that the study participants had pre-existing heart disease. In other words, taking statins in this case was more treatment than prevention.  The results were fantastic.  So fantastic, in fact, that the researchers ended the study early for ethical reasons; the drug supposedly saved so many lives that they thought it unethical to continue.  The drug did reduce heart-disease-related deaths dramatically... it even reduced all-cause death.  But there was one group in which the results werent so peachy... women.

Effect of atorvastatin vs. placebo on risk of death from heart disease

Yup, sticks out like a sore thumb... the women who took the placebo fared better than those who took the drug!  And this is for heart disease only... the data for all-cause death would inevitably be skewed more to the placebo side.  Unfortunately theres no handy dandy chart for that.

Surely this an aberration Burn!! My doctor would have told me about this! Nope, sorry. Its real. Two major meta-analyses have confirmed this finding (7, 8). In women without pre-existing heart disease, the drugs have no benefit whatsoever and likely increase all-cause death.  And in women who do? Well statins tend to reduce their risk of dying of a heart attack, but dont change their risk of dying overall.  In other words, they increase the risk that youll die of something else. I dont know about you, but I care more about overall survival than arbitrarily lowering my risk of just ONE mode of death.

Its clear that if youre a woman, regardless of whether or not you have heart disease, youre gaining nothing by taking statins. Except maybe an early grave. Or some wacko neuromuscular side effects. Dont do it.  And if your doctor insists on it? Send him/her here. Argument over.

Synthesis
Now, synthesize all of this information in your mind and what do you get??  Cholesterol levels in women just arent an issue.  I dont think its something we need to concern ourselves with. Again, each case should be evaluated on its own, but Ive talked to far too many women with "high cholesterol" who are unnecessarily worried for their health.  They come back from the doctor with a cholesterol level of 230, thinking they have to change their entire life around to fix it. FIX WHAT?? PLEASE TELL ME. And if he/she wants to get you started on statins? Time to find a new doctor, Toots, because this one obviously doesnt have a frickin clue.

Ill just leave you with this thought... Remember that time you heard you shouldnt eat saturated fat because it raises your cholesterol? Seems kind of irrelevant now, doesnt it? Dont fucking worry about it. Just dont.

Telephonic Nurse Led Disease Management Saves Money Again

They did it again!
Before too long, the Disease Management Care Blog will start getting bored with the accumulating evidence (for example, here, here and here) that telephonic disease management (DM) reduces medical claims expense in excess of program costs.  Theres only one thing thats more tiring, and that is the lingering ideological assumption among many academics that the Medicare programs inability to save money with DM means theres a problem with DM and not with Medicare.

Enter this study on diabetes telephonic care management that was just published in the American Journal of Managed Care.  This was about as real world as you can get.

The Denver Healths Westside Family Health Center, a federally funded community health center, randomly assigned 381 patients with diabetes to a 20-month telephone outreach program and another 381 to usual physician care.  In the intervention arm, patients not only saw their physicians, but had the involvement of 3 nurses sharing a ".75 full time equivalent" position.  The nurses used motivational interviewing and clinical protocols to guide these patients in self-care.  The nurses also independently ordered labs, initiated and titrated medications (with the use of pre-signed prescriptions as necessary), arranged vaccinations and facilitated physician appointments.  The nurses focused on control of lipids, blood glucose control and treatment of high blood pressure.

 Since many of these patients had no insurance, linear regression models were used to evaluate costs "from the perspective of the health care system" with "cost to charge ratios."  Patients with no primary care visit in the year preceding the end of the study were excluded from the analysis and the use of health care services outside of the Center was not counted. 

Aside from a slight gender imbalance (64% vs. 57% female in the intervention and control groups, respectively) as well as different rates of cerebrovascular disease (14% vs. 9%) and use of insulin (24% vs. 38%), the two patient populations were mostly similar in multiple baseline labs and co-morbid conditions.  In terms of the baseline costs, the control group had higher baseline hospitalization rates and total costs.

At the end of the study, 58% vs. 47% had achieved a target cholesterol LDL, but there was no difference in blood glucose or blood pressure control. 65 patients were lost to follow-up. The remainder were contacted an average of three times over the course of the study.

There was a significant drop in costs in the intervention group.  Over 18 months, their costs decreased from a baseline of $6390 per patient to $6217, while the control group went up from $7694 to $9033.
When the cost of the program was rolled in, the intervention resulted in $60,902 in total savings, which included the costs of $134,750 for the nurse-based intervention.  The savings appeared to be due to fewer hospitalizations.

While this study didnt use remotely positioned telephonic disease management and instead used nurses that were on-site, the DMCB points out that many telephonic programs use the same playbook. If the nurses have a good working relationship with the clinic physicians, they can do great things by telephone.  Last but not least, while the nurses pursued lipids, blood sugar and blood pressure, the DMCB suspects the real secret sauce was the countless and immeasurable other pieces of advice and help that were provided to the intervention patients.

Two New Studies Address Salt Intake And Cardiovascular Disease

First study:

Fatal And Nonfatal Outcomes, Incidence Of Hypertension, And Blood Pressure Changes In Relation To Urinary Sodium Excretion, JAMA, May 2011

While an increase in sodium (as measured by sodium excretion) was associated with an increase in systolic blood pressure (but not diastolic blood pressure), "this association did not translate into a higher risk of hypertension or CVD complications."

Also, from news@JAMA which summarized the study:
"In the study, the researchers categorized the 3681 study participants according to sodium intake, using a procedure that measures sodium in the urine over a 24-hour period, considered the gold standard for such assessment. At a median of nearly 8 years later, participants in the group with the lowest sodium intake at the beginning of the trial were significantly more likely than the other 2 groups to die of cardiovascular disease."

Second study:

Reduced Dietary Salt For The Prevention of Cardiovascular Disease: A Meta-Analysis of Randomized Controlled Trials (Cochrane Review), American Journal of Hypertension, July 2011

Results:
“Relative risks (RRs) for all-cause mortality in normotensives and hypertensives showed no strong evidence of any effect of salt reduction CVD morbidity in people with normal BP.

Raised BP at baseline also showed no strong evidence of benefit.

Salt restriction increased the risk of all-cause mortality in those with heart failure.”
Neither of these studies lend strong support for the recommendation to restrict sodium ... for cardiovascular disease. However, other conditions may benefit from sodium restriction. High sodium intake has been associated with greater calcium losses and osteoporosis.1 Also, the efficient renal mechanism that excretes excess sodium may not be fully effective if the kidney is compromised - as it is with diabetes, hypertension, or existing kidney and vascular disease.

Platelet clumping can cause endocarditis, or inflammation of blood vessels that can block the blood supply to the heart and brain.

Dr Helen Petersen, said, with a better understanding of the relationship between bacteria and the risk of blood clots, it can help especially in the treatment of infective endocarditis do. Infective endocarditis is an infection of the heart valves, which are usually caused by bacteria.

"About 30 percent of people with infective endocarditis will die and most require heart valve replacement surgery who have been infected, with valves made ​​of metal or animals," says Dr. Petersen.

April is Irritable Bowel Syndrome Awareness month. One of the conditions associated in some people with IBS is Gluten allergy. It is a highly under-diagnosed condition, therefore worth spreading public awareness about it. Gluten is a Protein, which can be found in Wheat, Barley, and Rye. The disease associated with gluten intolerance or allergy is called Celiac disease.

Celiac disease is an inflammatory condition of the small intestine that affects some people with genetic predisposition. The inflammationi damages the mucosa of the small intestine, which results in a lot of the symptoms and deficiencies associated with gluten allergy. About a third of Caucasians are genetically predisposed to Celiac disease, but not all of them get Celiac disease. Usually avoidance and exclusion of Gluten resolves symptoms of Celiac disease.

Women are twice as likely to be affected and diagnosed with Celiac disease than men. However, unless gluten is excluded, taking vitamins or minerals wont result in reversing the disease or brittle bones.

Gluten Allergy can be diagnosed through blood tests, endoscopy with biopsy of the duodenal mucosa, and genetic testing. However, if youre already on a gluten free diet, genetic testing is the only test, which will show Gluten allergy.

If you have celiac disease, lifelong avoidance of gluten is strongly recommended. Also you need to have your levels of vitamins and minerals like Vitamin D, B12, folic acid, zinc and copper checked frequently. There are a lot of support groups for people with Celiac disease and gluten allergy as well as a general public awareness and availability of Gluten free products in a lot of mainstream supermarkets. Visiting a dietitian and/or a physician who has expertise dealing with people with Celiac disease is also highly effective.
